    As the others say a very very brief pause in pedalling is all that is needed, never been an issue for me even on 25% gradients.

    Mine does it sometimes and sounds like it is at the front, I've also had my chain tensioner loosening and then skipping teeth on the rear sprocket with high pedal pressures, but I imagine yours is running without a tensioner so unlikely to be that.

    As you say ME/CFS is a waste bin diagnosis, and if your test is negative I expect mine will be too, I found one tick on my leg which didn't look to have bitten and one crawling across my clothes.

    Lyme Disease brought the thread back from the dead ;) not to say that the thread shouldn't carry on though, as most is just as relevant now as back when the thread was started, the bikes might have changed a little bit though.
