2nd one possible slippery manhole cover, been there but never at speed so never fell off, just kicked out a little. I get the feeling most are throttle use and most are contact with wet metal + more like motor bikes the are "E".

All bar two were throttle types(tried not the snigger) Offroad - to be expected, but that last one WTF, it was a clear road, no turns, no nothing, so i guess something mechanical happened.
1st one - couldn't take the turn, looked nasty and there always seems to be a concrete kerb right where you dont want it. Just too fast.
Tram rails - absolute killer, caught me out a couple of times in Amsterdam. Cross them at your peril.

Boredom. The only time I fell asleep cycling was on a long straight road in Lincolnshire... mile upon mile of cabbage fields... carrots... cabbages... carrots... cabbages... you are feeling very sleepy....
