Steering head adjustment

With apologies if this has been covered elswhere (I can't find anything) but, could someone please explain the correct procedure for adjusting a threaded steering head?

Loosen it, adjust it and tighten it, then tighten the locknut.

Is there something specific you want to do with it?

I apologize for hijacking this thread, but i was wondering if this was related to the same problem i have.
My handlebars are slightly off centre when the wheel is straight.
I loosened the two hex bolts as seen in the picture, but i still couldn't move the bars to align straight.
Is there another bolt under the top bit? It looks like a cap but i didn't want to prise if off in case it's not meant to be removed.

That's a threadless headset and I've never had anything that modern.

My steering head was loose (fore and aft play) but my problem is, having read conflicting information on the internet, I'm not sure just how tight the head nut should be. I've already tried tightening the lower nut to a degree which seems excessive to me but, although there's no more play, the steeing does not 'stiffen' as expected, so I don't know where to stop - if you understand me.

Ah, I don't know why it's not going stiff but the aim is to stop the play without it being stiff. I'd be wary of doing it too tight in case something breaks.
